A signed and dated waiver form is required of every player. 

 

Minors require the signature of a parent or legal guardian. Each waiver form is valid on the date of signature only. Waiver forms are also available at the park to be completed in-person by those over 18, or on behalf of a minor if you are the parent or legal guardian.
